Medication Forms and Action Plans
By NJ State law, students are not permitted to carry medications (prescription or non prescription over the counter). Any medication to be administered during the school day, must be prescribed by a physician and administered by the school nurse, after the required written approvals have been obtained. All medications must be in their original container. Medications cannot be accepted in baggies or unlabeled containers. The form for this approval is available in the Health Office or may be downloaded from this page. Only students with " life threatening " conditions such as diabetes, asthma, or anaphylaxis, may be permitted to carry and self administer their own medication after the required written approval has been obtained.
