Incoming 6th Grade Required Immunizations

All incoming 6th grade students are required by the State of New Jersey to have one dose of Tdap vaccine given no earlier than their 10th birthday, and one dose of Meningococcal vaccine given no earlier than their 11th birthday. These requirements MUST be met to enter school in September. Please see the required form below.

*Please Note- Letters with additional information about August/September birthdays will be mailed home over the summer.

Required 6th Grade Immunizations-Spanish

 

The New Jersey Department of Health (NJDOH) will not waive the vaccine requirements nor will there be any extensions for this school year. Students are expected to comply with all vaccine requirements prior to attending virtually or inperson at a NJ school.
