Athletic Forms
Any student interested in participating in Middle School sports must have a current physical exam on file. The exam must be less than one year old. The sports packet must be completed in its entirety and returned to the school nurse for review.
Please note the following important information regarding sports physicals.
- All physicals must be completed using the updated forms mandated by the State of NJ. All required forms are below.
- History questionnaire, to be completed by parent/guardian and student and reviewed by the doctor completing the physical. Make sure that the page is signed by a parent/guardian.
- If any question is answered "Yes", please explain in the section provided as to when and what.
- For ANY cardiac conditions, please include cardiac clearance from a specialist.
- Vision must be documented and if the student's vision is worse than 20/40, an eye doctor must be seen before the sports form is submitted. If your child has glasses please bring them to the doctor's appointment.
- Physical Examination Form and Clearance Form to be completed by the physician/health care provider. This form must have the physician’s signature on it.
- No other forms will be accepted. For example, the Universal Child Health Record is not the correct form.
- All physicals will be reviewed by the school physician, per regulation. Therefore, they must be submitted by the assigned school deadline.
- Please schedule your child's appointment in order to meet the DEADLINES.
- Late paperwork may cause a delay in participation.
Attention: Update as of June 1st 2024 paperwork has changed.
The attached forms must be completed prior to the start of any middle school sports. The due dates will be announced as each sport is introduced and a parent permission slip is returned. Forms must be completed prior to every sport. A full physical exam must have been done within 365 days prior to the start of the sport. If you have already turned in a packet and the physical is still within that time frame, only the permission slip and Update Questionnaire need to be completed by the parent.
All sections must be completed and signed before the packet will be accepted.
